Partial Rotator Cuff Tear: Collagen Patch Regeneten

How a biocompatible collagen patch rebuilds the tendon. Regenerative treatment possible without surgery.

Dr. Rhee Dong Kyu·7 min read

Regeneten is a bioabsorbable collagen matrix placed on the outer (bursal) surface of a rotator cuff tear. It represents a category of treatment distinct from injection therapy and distinct from surgery — a physical scaffold that supports tissue regeneration while providing immediate mechanical reinforcement.

Why bursal-side tears need a different approach

Bursal-side tears occur on the outer surface of the rotator cuff. Unlike articular-side tears (which can be treated with plication suture anchoring to the bone below), bursal-side tears benefit from reinforcement applied to the outer surface. Regeneten's placement directly on the thinned or torn outer tendon surface provides immediate structural support and a biological scaffold for cell ingrowth.

The two mechanisms of Regeneten

  • Mechanical: The patch physically reinforces the thinned/torn area, preventing further tear extension and distributing mechanical load across a larger surface area
  • Biological: As a collagen matrix derived from porcine small intestinal submucosa (SIS), Regeneten stimulates cell ingrowth, new collagen synthesis, and gradual replacement by autologous tissue

The evidence: >70% tendon thickness increase

Clinical studies and real patient ultrasound data confirm that Regeneten augmentation leads to significant increases in tendon thickness — typically >70% compared to pre-procedure measurements. This structural change is not just symptomatic improvement; it represents genuine tissue augmentation confirmed on imaging.

Best combined with plication suture

For tears that have both articular-side and bursal-side involvement — or for comprehensive reinforcement — combining plication suture (articular-side fixation) with Regeneten (bursal-side augmentation) provides the most complete structural restoration possible without open surgery.
