Why the period when pain decreases is the most dangerous time. What to do for recovery without relapse.

Of all the patterns seen in shoulder pain patients, one is particularly consistent and predictable: patients who improve often relapse faster than those who progressed slowly. The reason is almost always the same — when pain decreases, activity increases, and the underlying structural problem is re-aggravated before healing is complete.

The relapse cycle

1. Shoulder pain develops → patient seeks treatment → 2. Pain decreases with initial treatment → patient feels better → 3. Patient resumes full activity: gym, overhead work, lifting → 4. Pain returns, often worse than initial episode → 5. Patient returns, cycle repeats.

What happens structurally during a 'good day'

When inflammation is controlled by treatment, pain decreases. But if a partial tear exists, it has not repaired itself — it is simply less inflamed. Resuming heavy activity at this point places mechanical stress on a still-damaged tendon, causing micro-trauma that re-triggers the inflammatory cascade. The result is often a larger tear and worse inflammation than before.

  • Do not use pain level as a guide to activity resumption
  • Follow the structural recovery timeline, not how you feel
  • After plication suture: arm sling for minimum 2–4 weeks regardless of pain level
  • After regenerative injection: avoid overhead loading for the prescribed period

The shoulder that feels better is not the shoulder that is healed. Let the structure guide your return to activity.
