Calcific tendinitis is one of the most satisfying conditions to treat in orthopedics — because complete, durable resolution is achievable in virtually all cases with the right treatment. Understanding the three phases of calcific tendinitis

Calcific tendinitis is not static — calcium deposits evolve through distinct phases. In the formative phase, calcium is soft and paste-like. In the resting phase, it hardens into chalk-type dense calcium. In the resorptive phase, calcium liquefies as the body attempts natural reabsorption. Treatment must be matched to the phase.

1st Generation: Lavage (Needle washout)

Saline is injected into the calcium deposit to dissolve and flush it out. Works only for soft, early-phase calcium. Completely ineffective for hardened calcium. Often requires multiple sessions and leaves calcium debris.

2nd Generation: Barbotage (Needling and aspiration)

A needle is used to needle the calcium repeatedly while aspirating. More aggressive than lavage, but still limited to partially soft calcium. Hardened chalk-type calcium cannot be aspirated. Carries some risk of tendon damage with repeated needling.

3rd Generation: Crushing Aspiration (Platinum Technology)
