3 Things You Must Do First When Your Shoulder Hurts

Pain check, the right order of testing, and what you need to know before searching for treatment options.

Shoulder pain is not all the same. The location of pain, when it occurs, and how it feels tell a very different story depending on the underlying cause. Yet most people do the same thing when their shoulder hurts: search the internet for 'shoulder pain treatment' and make an appointment at the nearest orthopedic clinic.

Step 1: Identify the nature and location of the pain

The first thing is not to rush to a clinic, but to observe your own pain carefully. Is the pain sharp and stabbing, or dull and aching? Does it occur when you raise your arm, when you lower it, or constantly even at rest? Is it worse at night? Does it radiate down the arm?

  • Sharp stabbing pain: may indicate calcific tendinitis or acute inflammation
  • Dull aching pain with movement: suggests rotator cuff tear or impingement
  • Night pain waking you from sleep: classic calcific tendinitis presentation
  • Pain radiating down the arm to fingers: possible cervical nerve involvement

Step 2: Don't jump straight to treatment — get the right diagnosis first

The most common mistake is skipping proper diagnosis and going straight to treatment. 'Let's try shockwave therapy first' or 'Let's try some injections' — without knowing exactly what structure is damaged, treatment becomes guesswork. A proper diagnosis requires imaging: at minimum X-ray and ultrasound, and MRI when needed.

Treatment without diagnosis is just an experiment. The right diagnosis is the starting point for all treatment.

Step 3: Before searching 'treatment options' online, understand your condition

Internet searches for shoulder pain treatments will inevitably lead you to shockwave therapy, prolotherapy, PRP, stem cells, collagen patches — the list is endless. But none of these matter until you know exactly what you have. A calcific tendinitis patient and a rotator cuff tear patient may both have 'shoulder pain', but they need entirely different treatments.
