X-ray, Ultrasound, or MRI — Which Test Comes First?

The right order of testing based on cost and accuracy. Why you should always request the MRI report.

When shoulder pain starts, most people go to a clinic and get an X-ray. But is X-ray the right first test for shoulder pain? The answer is: it depends. Each imaging modality has its strengths and appropriate uses.

X-ray: The necessary first step

X-ray shows bone structure clearly and can detect calcific deposits (calcific tendinitis), bone spurs (acromion morphology), and gross structural abnormalities. It is the standard first step in any shoulder evaluation. However, X-ray cannot show soft tissue structures — tendons, ligaments, bursa, and cartilage are invisible on X-ray.

Ultrasound: The real-time tendon viewer

Ultrasound is the most valuable test for shoulder tendons. It shows the rotator cuff in real time, allowing dynamic assessment of tendon movement. Ultrasound can detect partial tears, full tears, bursitis, and fluid collections with high accuracy — and crucially, the doctor can perform the test while actively moving the patient's arm to identify impingement patterns.

MRI: The gold standard for deep structure assessment

MRI shows everything: tendons, cartilage, labrum, bone marrow, muscle quality. It is the only test that can reliably assess muscle atrophy, fatty degeneration, and labral tears (Bankart lesions). However, MRI is expensive, time-consuming, and not always necessary for straightforward cases.

Why always request the MRI report

If MRI was performed at another clinic, always request the actual report — not just a verbal summary. The radiologist's written interpretation contains details that a physician's verbal summary may omit or simplify. When seeking a second opinion, the written report is essential.
