Excruciating Shoulder Calcific Tendinitis: What Is the Fundamental Treatment?

Hello. I'm Dr. Dongkyu Lee, an orthopedic specialist. So many patients suffer from shoulder calcific tendinitis. Yet some don't receive proper treatment. The fundamental treatment for shoulder calcific tendinitis is removing the calcium deposit. From the stories patients tell me: Some only received pain injections for temporary relief. Some received 20-30 sessions of shockwave therapy even when the size and characteristics of their calcium deposit clearly wouldn't respond to shockwave alone. Pain injections and shockwave therapy have their place. But to fundamentally treat shoulder calcific tendinitis, removing the calcium deposit is the essential treatment. Today's case is a patient who suffered from shoulder calcific tendinitis for 3 full years. The patient was diagnosed with calcific tendinitis and received only shockwave therapy for 3 years. When the pain became severe, injections were added... Let's look at the X-ray:

The calcium is in the subscapularis and overlaps with the humerus on X-ray, making it hard to see. However, the area outlined by the red dotted line shows a significantly large calcium deposit.

The dark area within the yellow circle at the upper portion of the subscapularis is the calcium deposit. Surrounding white areas indicate inflammation.

By the time this patient came to me, she had endured as much as possible but the pain had become so severe that she couldn't move her arm, causing great distress. NewPIMS treatment (calcium crushing and aspiration), a non-surgical treatment for calcific tendinitis, was performed.

The white material in the syringe after the procedure is the crushed and aspirated calcium.

Post-procedure X-ray shows the calcium has been almost completely absorbed.

After viewing the post-procedure images, the patient said it felt refreshing to see the calcium gone and wondered why she had suffered for 3 years... She was extremely satisfied as the pain was also completely gone. Some patients avoid surgery out of fear and rely only on injections and conservative treatment. But as with any condition, early treatment keeps things simple — allowing the disease to progress makes treatment more complex and can worsen outcomes. Shoulder calcific tendinitis can be treated without surgery.
